Please attach a current demographics sheet, copy of Insurance card, Authorization if indicated, most recent visit notes, any other relavent studies and / or tests pertaining to this referral.

Pulmonary Referrals

For Pulmonary Consultations and Pre-Op Clearances we request an authorization be submitted for a Spirometry and a Pulmonary Exercise Test / 6- Minute Walk to accompany the consult code.

Adding these codes will enable our staff to perform these tests at the initial visit if needed, and can save patients an additional visit.

Sleep Medicine Referrals

For Sleep Medicine consults please authorize a consultation code.
